If you are swiping the card and it is continually failing, one thing to check would be if it is an issue with the terminal you are using. 

Most types of ICC cards are dual interface cards and some terminals, such as the Ingenico icm122, appear to have poor shielding to prevent the terminal from reading the chip before the swipe has been completed. We would suggest you talk to your terminal vendor about this issue as it may be a known issue.
